Heart Bridge was born out of a shared realization: while the modern healthcare system excels at treating the body, it often leaves patients and families to navigate the emotional and logistical realities of severe illness and the dying process entirely on their own. We saw a critical need for an organization that could speak the complex language of medicine, while remaining fiercely dedicated to the practical, deeply human work of simply holding your hand and walking with you through life’s most difficult transitions. Today, Heart Bridge serves San Francisco and San Mateo Counties through two integrated missions: providing direct, compassionate end-of-life navigation for our community, and offering high-level clinical and holistic mentorship for the next generation of end-of-life doulas.

We built Heart Bridge because we believe that no one in our community should have to face aging, illness, or end-of-life planning alone. Whether we are hosting an advance healthcare directive clinic at a local senior center, mentoring a new cohort of doulas, or sitting quietly at a bedside, our mission remains the same: to provide clarity, calm, and unwavering support when it matters most.
